An Iowa checklist for updating your records

Your name change is official. For many people, that moment brings joy, relief, and a new question: where do I start with updating everything?

Lavender Legal Center’s reel slides walk you through the next steps, from protecting your court decree to updating the accounts you use every day. Use them as a visual checklist and return to this guide as you work through the process.

Start with your name-change decree.

Keep a certified copy of your Decree of Name Change in a secure place, and consider ordering extras from the clerk of court. Some agencies require certified proof rather than an ordinary photocopy. Iowa courts provide a certified copy when a name change is granted. Iowa Judicial Branch

Update Social Security before your Iowa ID.

Start with the Social Security Administration so other agencies can verify your new name against federal records. Iowa Attorney General

You’ll request a replacement Social Security card showing your new legal name; your Social Security number stays the same. Depending on your circumstances, you may be able to complete the request online or need an appointment. Start with Social Security’s name-change tool, and review its card guidance.

Next, update your Iowa driver’s license or state ID.

Plan an in-person visit and bring certified proof of your name change. Iowa DOT does not accept photocopies or faxes of name-change documents and currently lists a $10 replacement fee. Check the requirements before your visit so you arrive prepared. Iowa DOT

Check on your birth record.

For an Iowa birth record, the court clerk sends an abstract of the qualifying name-change decree to Iowa Health and Human Services. Contact Vital Records about obtaining an updated certified copy. If you were born elsewhere, check the requirements in the jurisdiction where your birth was recorded. Iowa Code, Chapter 674

Work through your other records and accounts.

Our slides include a checklist of places to review, depending on your circumstances:

  • Passport and voter registration

  • Employer and payroll records

  • Immigration documents

  • Veterans or public-benefit records

  • Bank accounts, credit cards, and loans

  • Insurance and health-care records

  • School records and professional licenses

  • Lease, mortgage, and utility accounts

  • Email, subscriptions, and memberships

Each organization may have its own process. Check what it requires before sending original documents.

As you go, track the date of each request, the documents you submitted, and any confirmation number.

Mark an update complete once you receive confirmation that your new name appears correctly.

There can be a lot of paperwork after a name change. Having a checklist gives you a place to start—and a way to see your progress.
